Больницы Пекина и северного Китая борются с резким увеличением числа детей с респираторными заболеваниями, пока страна вступает в свою первую зиму после ослабления строгого контроля за COVID-19 почти год назад.
Время ожидания приема у врачей растягивается на часы, сотни пациентов стоят в очередях в некоторых детских больницах в крупных городах северного Китая, согласно сообщениям CNN, а также китайским государственным и социальным сетям.
Представитель Пекинской детской больницы сообщил государственным СМИ во вторник, что в настоящее время в среднем более 7000 пациентов ежедневно “намного превышают пропускную способность больницы”. Крупнейшая педиатрическая больница в соседнем Тяньцзине побила рекорд в субботу, приняв более 13 000 детей в своих амбулаторных отделениях и отделениях неотложной помощи, сообщает местное государственное издание.
Когда в четверг CNN позвонила, чтобы узнать о времени приема в Пекинской больнице дружбы, сотрудник сказал, что прием у педиатра может занять весь день.
“Прямо сейчас у нас здесь много детей. Те, кто записался на экстренный прием вчера, все еще не смогли попасть к врачу сегодня утром”, — сказал сотрудник.
Чиновники здравоохранения Пекина и других крупных городов на севере Китая заявили, что основными причинами были типичные сезонные заболевания, включая грипп и респираторно–синцитиальный вирус (РСВ), а также микоплазменная пневмония – бактериальная инфекция, которая обычно вызывает легкую форму заражения и обычно поражает детей.
Резкий рост числа случаев заболевания на севере Китая происходит на фоне роста сезонных респираторных инфекций по всему северному полушарию, в том числе в Соединенных Штатах, где РСВ распространяется на “беспрецедентных” уровнях среди детей, отмечает CNN.
Но ситуация в Китае вызвала глобальную обеспокоенность после того, как Всемирная организация здравоохранения (ВОЗ) в среду попросила Китай предоставить больше информации о росте респираторных заболеваний и “сообщила о скоплениях недиагностированной пневмонии у детей”, ссылаясь на сообщение системы эпиднадзора ProMED с открытым исходным кодом.
Однако после беседы с представителями министерства здравоохранения и больниц Китая в четверг ВОЗ заявила, что данные указывают на увеличение числа амбулаторных консультаций и госпитализаций детей из-за микоплазменной пневмонии в мае и распространенных сезонных заболеваний RSV, аденовируса и вируса гриппа с октября.
”Некоторые из этих повышений происходят в начале сезона раньше, чем это было исторически, но не являются неожиданными, учитывая отмену ограничений на COVID-19, которые аналогичным образом наблюдались в других странах», — заявили в ВОЗ.
Агентство добавило, что китайские власти заявили, что “не было обнаружено каких-либо необычных или новаторских патогенов или необычных клинических проявлений”.
Внешние эксперты, наблюдающие за ситуацией, также отметили, что не было никаких свидетельств действия нового патогена, но призвали Китай поделиться дополнительной информацией о ситуации с общественностью.
“Мы не думаем, что где-то спрятан неизвестный патоген”, — комментирует CNN Цзинь Дуньянь, вирусолог из Школы биомедицинских наук Университета Гонконга. “Этому нет доказательств”.
Кэтрин Беннетт, эпидемиолог из Университета Дикин в Австралии, сказала, что главная проблема заключается в том, не указывает ли рост заболеваемости детской пневмонией на новый патоген или новые уровни тяжести заболевания.
“Пока мы не слышали сообщений ни о том, ни о другом”, — подчеркивает Беннетт, добавив, что важно отслеживать источники заражения, чтобы исключить подобные опасения.
В последние недели китайские родители жаловались в социальных сетях на переполненность больниц, где детям требуется несколько часов, чтобы обратиться к врачу, а затем еще более длительное ожидание сдачи анализа крови или внутривенной капельницы.
Учитывая относительно слаборазвитую систему первичной медико-санитарной помощи в Китае, больные люди обычно направляются в больницы или отделения неотложной помощи в качестве первого пункта контакта. В пик сезона эти учреждения могут быть переполнены.
На китайской платформе социальных сетей Weibo широко распространилась фотография, на которой был изображен больничный экран, уведомляющий пациентов о том, что в очереди более 700 человек, а предполагаемое время ожидания составляет 13 часов.
В педиатрической больнице, связанной со Столичным институтом педиатрии в Пекине, залы были настолько переполнены, что некоторые дети с капельницами сидели на коленях у своих родителей, которые выстроились вдоль коридоров на складных табуретках, как показали видеоролики в социальных сетях.
Национальные органы здравоохранения Китая и должностные лица больниц неоднократно призывали родителей не отправлять детей напрямую в крупные педиатрические учреждения, вместо этого призывая их отвозить детей на диагностику в другие медицинские центры, предлагающие первичную медицинскую помощь или услуги общего профиля.
Национальная комиссия здравоохранения (NHC) предупредила родителей в четверг, что в крупных больницах может быть “длительное время ожидания и высокий риск перекрестного заражения”, направляя их в другие учреждения для сортировки. В заявлении NHC говорится, что он проинструктировал “все населенные пункты” усилить свои системы ведения пациентов и лечения, включая выявление тяжелых случаев среди наплыва пациентов.
Муниципальное правительство Пекина тем временем опубликовало статью в государственных СМИ, в которой врач говорит родителям, что им не нужно просить о внутривенном введении жидкости, “как только у ребенка поднимется температура”.
ВОЗ в четверг заявила, что китайские официальные лица сообщили, что “рост числа респираторных заболеваний не привел к тому, что нагрузка на пациентов превысила возможности больниц”.
Всплеск посещений больниц совпадает с первой полной зимой в Китае без контроля “нулевого уровня Covid”, когда люди соблюдали строгую социальную дистанцию и носили маски для лица. Меры контроля были резко ослаблены в декабре прошлого года после того, как вспыхнули редкие протесты против мер по борьбе с пандемией, которые включали строгие карантины.
Неясно, произошло ли увеличение числа респираторных заболеваний или тяжелых случаев среди детей по сравнению с докандемическими годами из-за ограниченных общедоступных данных, опубликованных Китаем.
“Во время нулевого COVID эти распространенные респираторные заболевания были недооценены, поскольку люди избегали больниц, и поскольку все практиковали некоторое социальное дистанцирование, заболеваемость была низкой, — говорит Цзинь, вирусолог из Гонконгского университета. – Абсолютно нормально, что в этом году по сравнению с прошлым годом будет большой рост. Но будет ли это большой скачок по сравнению с 2018, 2019 годами, это еще предстоит определить”.
Цзинь добавил, что в текущей ситуации могут играть роль социальные факторы, поскольку родители также могут быть больше обеспокоены здоровьем своего ребенка после пандемии, что побуждает больше людей обращаться за медицинской помощью.
Больше внимания уделяется вспышкам заболеваний после появления пандемии коронавируса в конце 2019 года. Также раздаются призывы к большей прозрачности – в том числе из Китая, который обвиняется в препятствовании расследованию происхождения вируса и сокрытии ранней информации о его распространении.
Кристин Дженкинс, профессор респираторной медицины в UNSW Sydney, сказала, что рост числа вирусных инфекций дыхательных путей у детей в это время года не является неожиданностью и является явлением, которое наблюдалось в течение многих десятилетий во всем мире с наступлением зимы.
“Однако в контексте пандемии, вызванной относительно новым вирусом, таким как новый коронавирус, и потенциальной возможностью того, что другие новые вирусы или мутации могут вызывать заболевания дыхательных путей, необходимы оперативная отчетность и мониторинг”, — сказала она.
